    Here is a set of Western Electric 500 / 2500  series telephone jacks for modular handsets and cords  These break easily so its  best to keep some on hand.They fit the 500 rotary desk phone and the touch tone 2500 series.
    The line cord jack is a 623D and the handset jack is a 616D.
    Fits Stromberg Carlson and ITT also.
